I need help with my on-going or previous problem. What should I do?
If you are concerned about you're current condition, please call our office during our normal business hours. Our staff are available to assist you from 8-7 Monday and Thursday, and from 8-5 on Tuesday, Wednesday and Friday. Most patient questions are best handled when our physicians, hand therapists and medical assistants are in the office. Most routine patient inquiries can be handled quickly and easily. We're here to help you.

If it is after our normal business hours, and your concern is urgent and cannot wait until the next business day, please call our office where you will be routed to our after-hours answering service and a message will be directed to the physician on-call.

If you have a question about a recent surgery, please refer to our post-op information sheet and any additional paperwork given to you at the time of your surgery.

I am a patient of the Center and I have a NEW problem. What should I do?
The Hand Center of Waterbury treats all kinds of injuries and ailments of the hand, wrist, elbow, arm and shoulder. If you have a new problem, please call our office and schedule an appointment to see one of our physicians.

Existing patients with new problems will need to fill out the appropriate form to document the new problem.

  • If you are an existing patient and have been seen by a doctor at our practice within the past 6 months to 3 years, please fill out this form: New Problem Form (PDF)

What if I think my NEW problem is work-related or I was RE-INJURED at work?
The professionals a The Hand Center of Waterbury are equipped to assist in all forms of work-related injuries of the hand, wrist, arm, elbow and shoulder. We will work with you to determine whether your current condition could be caused or exacerbated by your work conditions. When it is known that an injury is work-related, we require all our patients to follow the Connecticut State Worker's Compensation guidelines and file a claim with their employer. Once a claim has been filed, we will contact your Worker's Compensation insurance carrier to obtain the appropriate authorization to treat your injuries.

Post-op Patient Information
If you recently had surgery with one of our physicians, please refer to the paperwork given to you at the time of your surgery for answers to common questions about your surgery. You may also view or print a copy of the instructions given to you by our office by clicking here. Non-emergent questions about your surgery can be addressed at your post-op appointment that is typically scheduled about 9-12 days after your surgery date.

For urgent questions and/or if you are still concerned about your surgery or surgical site, please call our office and we will be happy to help you.

Prescription Refills
All requests for prescription refills will be handled through the office medical assistants with direct communication with our physicians. Prescriptions will be refilled during business hours only, and will not be processed after business hours or on weekends. We will not honor any prescription refill requests by e-mail. Please call our office if you would like us to refill your prescription.

Getting copies of medical records
Our office follows federal guidelines of the Health Insurance Portability and Accountability Act (HIPAA,) and other privacy rules. All patients who would like a copy of their medical records should contact our office and complete a Records Release Form. Although Connecticut State Law requires copies of records be available within 30 days of a request, our office works hard to process all records requests within 7-10 days.

Connecticut General Statutes, § 20-7c, allows a charge of forty-five cents per page and the cost of first class postage, if applicable, for furnishing a health record. Additional fees may be assessed for copies of x-rays or other records.

Pursuant to this subsection, no charge shall be made for furnishing a health record or part thereof to a patient, his attorney or authorized representative if the record or part thereof is necessary for the purpose of supporting a claim or appeal under any provision of the Social Security Act and the request is accompanied by documentation of the claim or appeal.

Patients who are treating under an active worker's compensation claim are also not charged for copies of their records.

Getting forms filled out such as: disability forms or other legal forms
Our office gets numerous requests to complete disability and other forms for our many patients. To ensure that all forms are completed correctly, we require 7 to 10 days to process these requests. Please bring any forms you need completed to our office as soon as possible to best facilitate this process.
