What to expect from your first visit
We strive to provide excellent customer service to all our patients from their first phone call to the office to their last appointment. To help us help you make your first appointment a success, please bring the following to your appointment:
- insurance card,
- photo ID,
- co-pay or co-insurance payment,
- insurance referral if needed,
- copies of any other physician notes for the same problem,
- copies of nerve studies,
- copies of x-rays or imaging studies such as CT Scans or MRIs for the problem area, and
- your completed new patient intake form. Our new patient intake form can be downloaded here or we will be happy to mail or fax a copy to you at your request; simply call our office and our staff will be happy to help you Contact Us.
For our older patients or any patient who has support or help with medical decision making, we ask that you bring your family member or other support person with you to your appointments with our doctors.
For patients under the age of 18 (minors,) please refer to our policy on the treatment of minors.
Please also be advised that in adherence to our health insurance company contracts, our office policy is to re-schedule appointments for patients who are not prepared with their co-pay or co-insurance at the time of the appointment.

Were you injured at work?
The Hand Center of Waterbury has treated all forms of work-related injuries to the hand, wrist, arm, elbow and shoulder from repetitive stress injuries to traumatic crushing or amputation injuries. Our office staff are skilled in the processes of the Connecticut Worker's Compensation System and can assist our patients in getting the benefits they deserve. If you are unsure if your injury is related to your work, we will assist you in determining whether you will utilize your private health insurance or need to file a claim for Worker's Compensation benefits through your employer. Once a claim has been filed, we will contact your Worker's Compensation insurance carrier to obtain the appropriate authorization to treat your injuries. Our team of hand surgeons and hand therapists will work together to help you get back to the optimal functioning and return you to work as quickly as is reasonable for your injury.

The Worker's Compensation Process
- Report your injury to your employer immediately
- Get prompt medical attention
- File an official claim as soon as possible
- Continue medical treatment as advised by your physician(s)
- Follow work restrictions directed by your physician
- When maximum medical improvement has been reached, your physician will issue a medical report on Permanent Partial Disability (PPD)
- If your claim is contested or denied, you may request an Informal Hearing with the Worker's Compensation Commission (http://wcc.state.ct.us)
